Client File and Privacy Statement
According to the Dutch Medical Treatment Agreement Act (WGBO), as a therapist I am legally required to maintain a client file, which must be retained for 20 years after the last modification. This file contains notes about your health condition and information regarding the treatments I have provided. It may also include information received from other healthcare providers, but only with your explicit consent.
Your privacy is safeguarded through careful handling of both personal and medical data. As your treating therapist, I am the only person with access to your file, and I am bound by a duty of confidentiality.
Use of Your Data
Information from your file may be used for the following purposes:

Sharing information with other healthcare providers, but only with your explicit consent

Use during temporary replacement by another therapist

Anonymized use for peer review or intercollegial consultation

Use for financial administration, either by myself or a designated administrator

If I wish to use your data for any other reason not listed above, I will first inform you and request your explicit permission
Invoice Information
In order for your invoice to be eligible for insurance reimbursement, it will include the following details:

Your full name and address

Date of birth
-
Invoice date
-
Date of treatment
-
Type of treatment: Shiatsu (code 24102) and/or Acupuncture (code 24104)

Your Rights Regarding Your Data

You have the right to access, correct, or delete any personal data you have provided to me

You may object to the processing of your data (in whole or in part) by me or by any third party acting on my behalf

You also have the right to request that your data be transferred either directly to you or, upon your request, to another service provider

If I process your data based on your consent, you have the right to withdraw this consent at any time
Note
If you believe your data has not been handled correctly, you may file a complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
